I haven't spoken to a lot of ancillary medical professionals in the show and explored ways that they are changing the way we deliver medical care. Today I spoke to a physical therapist who chose to run his practice unlike traditional practices. He doesn't structure therapy based on what maximizes revenue from insurers but instead on treating the patient and their injury.
Instead of designing a treatment regimen and session revolving around different modalities that pay the best, he focuses on the injury or chronic condition. Joe Kik runs his own private practice with no back office staff to save on overhead.  Techs don't just run people on exercise machines to take up time in a therapy session. He also doesn't treat people for a set amount of time just to make sure he racks up the correct charges.
That is because Joe doesn't take insurance so he can do whatever he thinks will maximize the results for his patient that day. However, he will  provide patients with the codes if they want to submit to insurance it turns out that many times his service is cheaper because there are no hidden fees. Most patients don't even realize how expensive their physical therapy is until they get the bill. And because they pay towards a deductible and add in copays for ten sessions their overall bill might be higher.
The direct care model in medicine is not limited to just cosmetic surgery and for the ultra wealthy. It is starting to emerge as the way for paying for health care in the future. Direct care is still the minority way of getting care but it is growing quickly as clinicians find it more satisfying professionally and patients appreciate the personal nature of the care they receive.
